Transforming Episode Accountability Model (TEAM) Participant
Franciscan Health Michigan City was selected to be a participant in the TEAM (Transforming Episode Accountability Model) program. TEAM is an initiative for people enrolled in Medicare Parts A and B receiving select procedures, who also meet certain criteria. TEAM is designed to improve coordinated care from surgery through the first 30 days after you leave the hospital.
TEAM is designed to make your recovery smoother by improving communication between your hospital, surgeons and primary care providers. As part of the program, hospitals help connect you with primary care follow-up to support long-term health.
The goals of TEAM are to reduce complications, prevent emergency room visits or readmissions, and improve your overall recovery experience while helping keep health care costs down.
