CareLinks
The time immediately following your stay at the hospital is a crucial point of needing highly specialized care. In addition, you are also at the highest risk of being readmitted to the hospital. CareLinks is here for you during this time to help you bridge that care gap. We provide you a convenient location to receive a quick "check up" to keep your condition from worsening. We do not replace your primary care provider but rather serve as a link between you and your doctor at a critical time in your care.
CareLinks is open Monday through Friday, 7:30AM - 4PM. If you are calling CareLinks to provide a ride home for discharged patients, we can accept patients up until 3:30 p.m.
